2. Problems of bacterial contamination

Summary

Now as before, the most important factor in the development of postoperative disorders of wound healing is bacterial contamination. The percentage of the disorders in which staphylococcus aureus is the causative organism is smaller than is usually assumed. In contrast to this, anaerobic microorganisms are the bacteria which can be demonstrated most frequently. The latter are usually the true causative organisms of infections which have been designated as coliform infections on account of the unpleasant odor of the pus. Determination and elimination of unrecognized deficits in the aseptic surgical technique resulted in an impressive decrease of disorders of wound healing. Hitherto the role of organism carriers amongst the personnel has been overestimated as far as staphylococcal infections are concerned. After regular disinfection of beds the hospital strains which had until that time been dominating as causative organisms of wound infections, disappeared completely and disappeared almost completely when the personnel was checked. The failure of antibiotic prophylaxis against wound infections is explained by a comparison of the antibiotic tissue concentration to the minimum inhibition concentration of the most important organisms of infections.

The most important factor in the development of postoperative disorders of wound healing is still bacterial contamination. The proportion of Staphylococcus aureus causing these disorders is smaller than is usually assumed; On the other hand, the bacteria to be detected most frequently are anaerobes. The latter are usually the real causative agents of those infections that were previously referred to as coli infections due to the foul smell of the pus. Uncovering and eliminating undetected gaps in aseptic surgical technique led to an impressive reduction in wound healing disorders. The role of the germ carriers among the staff has been overestimated in staph infection so far. After regular bed disinfection, the hospital tribes that had dominated until then disappeared completely as pathogens for wound infections and almost completely when the staff were checked. The failure of antibiotic prophylaxis against wound infections is explained by a comparison of antibiotic tissue concentrations with the minimum inhibitory concentration of the most important infectious agents.
